Community involvement

Enhancing health in our community by looking forward and giving back

At Lakeview Hospital and HealthPartners Clinic in Stillwater and Somerset, we work every day to improve the health and well-being of our patients, families and the St. Croix Valley community.

Through the Lakeview Health Foundation and in partnership with local organizations, community members and volunteers, we’re able to help develop resources, identify programs and support initiatives that promote health and well-being for everyone.

Health and Well-Being Advisory Committee

Since 2011, the Health and Well-Being Advisory Committee of the Lakeview Foundation has served as the eyes and ears for Lakeview Health, identifying the health priorities for our community. The committee is the primary advisory council for all of our community health work, helping Lakeview Health to provide vital programs and services designed to meet the health needs of our community, many of which we describe below.

Community Health Action Team

The Community Health Action Team (CHAT) is a dynamic group of community and health care leaders that meets to collaborate and address unmet community health needs in our area. Our focus is on improving the health of our communities through action, networking and education.

CHAT has been meeting since early 1990 at Lakeview Hospital and continues to meet monthly on the first Friday of every month.

Health experts predict that children today will have less healthy lives than their parents – unless something changes. Lakeview Health Foundation is helping create that change head-on.

PowerUp is a community-wide health initiative inspiring kids and families to eat better, move more and feel good. We partner with schools, families, organizations and the entire community to create classes, events, activities, education and more. Together, we’re focused on empowering kids to reach their full potential.

Scholarships

The Lakeview Health Foundation’s scholarship program was originally established to assist local students pursuing an advanced education in medicine. The program has expanded over time to recognize the ongoing needs of both our community and our hospital for qualified, caring health care professionals in a variety of roles.

Health and well-being grants

To form partnerships that improve the health and well-being of our communities, we make grants to local organizations. Our grantmaking empowers our communities and builds momentum for innovative programs and services that make a direct impact on local health priorities.

Lakeview Health Foundation's board of directors has oversight of the disbursement of health and well-being grants on behalf of all Lakeview Health subsidiaries. Grants are paid out of Hospital funds, but all correspondence and proposals should be sent to the Foundation office.

Grant requests are reviewed once a year at the February meeting of the Lakeview Health Grants Committee. Please submit your request no later than December 16, 2022. View our 2021 Grant Application and guidelines (PDF), and our most recent Community Health Needs Assessment Summary (PDF).

One in five people will experience some kind of a mental illness in their lifetime, and many don’t get the help they need. Make It OK is a campaign to reduce the stigma around mental illness and make room for caring conversations.

Make It Ok Ambassadors reach out to the community to help make it ok to talk about mental health with presentations, resources and a listening ear. For information about becoming an ambassador, contact us at makeitok@healthpartners.com or find Make It Ok classes and trainings.

Through Make it OK, we also partner with our community to connect people to the mental health care and support they need. And at Lakeview Hospital and HealthPartners Clinic in Stillwater and Somerset, we work to make it simpler for people in our community to learn about the help that is available to them and how they can get access to mental health care right here, close to home.

Faith Community Nurse Program

The St. Croix Valley Faith Community Nurse Program is a health care ministry providing physical, emotional and spiritual care for local congregations in partnership with Lakeview Hospital. Faith Community nurses are Lakeview Hospital employees who serve area churches.

The role of the nurse varies from place to place, depending on the needs of the church. Nurses may serve as health educators, health counselors, volunteer coordinators, interpreters of the health-and-faith relationship, referral sources or health advocates.
